Build a provider list
Record each provider, location, service date, and the reason for the visit. Keep discharge instructions, referrals, test information, and work restrictions with the matching provider entry.
Keep bills and care records distinct
A bill shows a charge, while a medical record describes care. Save both, along with insurer explanations of benefits and receipts, rather than treating one document as a substitute for another.
Use dates instead of memory

Share records carefully
Medical files contain sensitive information. Confirm the recipient and use the firm or insurer’s stated secure process. Keep a copy of what you sent and the date it was provided.
